Abstract. Purpose:To report cases of bullous keratopathy that necessitated eventual keratoplasty. Cases and Method:This retrospective study was made on 30 eyes of 30 patients who received penetrating keratoplasty at Nagasaki University Hospital during 4 years through 2005. Results:Bullous keratopathy was caused by cataract surgery with corneal implantation in 9 eyes(30%), laser iridotomy for glaucoma 6 eyes(20%)and others. It was idiopathic in 8 eyes(27%). Interval from cataract surgery to bullous keratopathy averaged 37.4±44.1 months. Bullous keratopathy developed later after iridotomy for glaucoma than prophylactic iridotomy(p=0.017). The present series was characterized by diabetes mellitus and glaucoma as systemic and topical underlying lesions. Conclusion:Bullous keratopathy may be due to fragility of corneal endothelium secondary to glaucoma, diabetes mellitus, or intraocular inflammation. Antiinflammatory measures are recommended to prevent bullous keratopathy after intraocular surgery.
