頭蓋内に進展した涙腺原発と考えられるMALTリンパ腫の1例

A case of mucosa-associated lymphoid tissue（MALT）lymphoma of presumed lacrimal gland origin with intracranial invasion

要約　目的：涙腺原発と推定されるMALTリンパ腫が頭蓋内に進展した症例の報告。症例と所見：74歳女性が1年前から続く両眼の眼瞼腫脹と結膜浮腫で受診した。磁気共鳴画像検査（MRI）で頭部に腫瘍があり，両側の前頭洞，篩骨洞，蝶形洞，左涙腺背側部の眼窩内と鞍上部から左海綿静脈洞に及んでいた。両側の涙腺が腫大していた。左側涙腺腫瘍の生検でMALTリンパ腫の診断が確定した。全身検査で転移はなかった。化学療法を行い，以後20か月後の現在まで病状は安定している。結論：MALTリンパ腫は一般に悪性度が低いとされているが，これが涙腺に原発し頭蓋内に浸潤することがある。

Abstract.　Purpose：To report a case of mucosa-associated lymphoid tissue（MALT）lymphoma of presumed lacrimal gland origin with intracranial invasion. Case and Findings：A 74-year-old female presented with bilateral eyelid swelling and chemosis since one year before. Magnetic resonance imaging（MRI）showed a tumor involving both frontal,ethmoidal and sphenoid sinus,left cavernous sinus,and left orbit dorsal to the lacrimal gland. Both lacrimal glands were swollen. Biopsy of left lacrimal gland tumor led to the diagnosis of MALT lymphoma. There was no systemic metastasis. The patient has been doing well for 20 months until present following systemic chemotherapy. Conclusion：MALT lymphoma has been considered to have low malignancy. This case illustrates that it may originate from lacrimal glands with extensive intracranial extension.
