Abstract. Purpose:To report a case of mucosa-associated lymphoid tissue(MALT)lymphoma of presumed lacrimal gland origin with intracranial invasion. Case and Findings:A 74-year-old female presented with bilateral eyelid swelling and chemosis since one year before. Magnetic resonance imaging(MRI)showed a tumor involving both frontal, ethmoidal and sphenoid sinus, left cavernous sinus, and left orbit dorsal to the lacrimal gland. Both lacrimal glands were swollen. Biopsy of left lacrimal gland tumor led to the diagnosis of MALT lymphoma. There was no systemic metastasis. The patient has been doing well for 20 months until present following systemic chemotherapy. Conclusion:MALT lymphoma has been considered to have low malignancy. This case illustrates that it may originate from lacrimal glands with extensive intracranial extension.
