Cases of planned retention of intravitreal silicone oil

要約　　過去6年間に硝子体手術にシリコーンオイル充填術を併用した193眼のうち，17眼（8.8％）では，以下の理由によりこれを抜去しなかった。7眼は低眼圧であり，抜去による眼球瘻に至る可能性が高かった。4眼には術後に強い増殖性硝子体網膜症（PVR）と網膜剝離が生じ，抜去により全網膜剝離になる可能性が高かった。PVRの他の1眼は，抜去に続いてガスタンポナーデが必要であったが，術後の安静が不可能であった。5眼では眼球瘻がすでに発症していた。シリコーンオイルを抜去できない症例は，視機能のいかんにかかわらず，経過観察が必要である。

Abstract.　We performed vitreous surgery with silicone oil tamponade in 193 eyes during the past 6-year period. Intravitreal silicone oil had to be retained in 17 eyes（8.8％）because of following reasons. Ocular hypotony was present in 7 eyes. Removal of silicone oil appeared to induce phthisis bulbi. Proliferative vitreoretinopathy（PVR）with retinal detachment developed after initial surgery. Removal of silicone oil appeared to lead to total retinal retinal detachment. Another eye with PVR needed gas tamponade following removal of silicone oil. This patient could not keep a rested position necessary for this procedure. Phthisis bulbi was alreadu present in 5 eyes. We advocate close follow-up of patients with retained intravitreal silicone oil.
