Abstract. We performed vitrectomy for diabetic retinopathy with vitreous hemorrhage in the left eye of a 67-year-old male. He had received cataract surgery with intraocular lens(IOL)implantation 7 months before. A flexible iris retractor was used to enlarge the pupil during vitrectomy. Endophthalmitis with hypopyon developed on day 2 of surgery. The affected eye was treated by irrigation of vitreous cavity by a fluid containing antibiotics. No microorganism was isolated from the aqueous or vitreous fluid. Signs suggesting retinal detachment developed 5 days later. Consecutive surgery showed absence of retinal detachment or of retinal vasculitis secondary to endophthalmitis. No microorganism was isolated from the extracted IOL.This case illustrates that endophthalmitis is a liability after vitreous surgery for diabetic retinopathy,particularly when intracameral procedures are performed.
