A 45-year-old male presented with lens luxation and secondary glaucoma. He showed typical manifestations of Marfan syndrome. Lens luxation had persisted for 41months in the left eye and 24months in the right eye. When seen by us,both eyes showed dislocated lens in the anterior chamber and in apposition to the posterior corneal surface. The endothelial cell population was decreased at 606/mm2right and 1,282/mm2left. He has been doing well after receiving intracapsular lens extraction through self-closure scleral incision in both eyes. This case illustrates that due attention is necessary to the corneal endothelial cells in Marfan syndrome and that self-closure incision is minimally invasive to the corneal endothelium during lens extraction.
