A 71-year-old female presented with blurring of vision in both eyes since one month before. She had been diagnosed with pulmonary tuberculosis 5months before. Systemic treatment with three drugs,including ethambutol 750mg daily,had started 4months before. Her corrected visual acuity was 0.4 right and 0.3 left. Dyschromatopsia was also present. Both eyes showed normal fundus and visual field when tested by Goldmann perimeter. Ethambutol was immediately discontinued. One week after her visit,the visual acuity decreased to 0.1 in either eye. Two weeks later,Humphrey perimetry showed central scotoma and temporal constriction in both eyes. The visual acuity started to recover 3months after withdrawal of ethambutol and improved to 1.0 each after 11months. This case illustrates that optic neuropathy may develop following use of ethambutol at the dosis of 18.3mg/kg/day for 3months and that perimetry facilited its early diagnosis.
