A 70-year-old male presented with progressive symblepharon and difficulty in lid closure since 4 years before. His visual acuity was light perception right and hand motion left. Both eyes showed severe symblepharon and corneal stem cell deficiency. He had,additionally,adhesion of nasal mucosa and oral-pharyngeal ulcers. Direct immunofluorescent study of oral mucosa showed linear deposit of IgG and C3 in the basal membrane,leading to the diagnosis of mucous membrane pemphigoid(MMP). Despite systemic medications ,the left cornea perforated 6months later. The ocular surface was reconstructed by combined lamellar keratoplasty,limbal transplantation,and amniotic membrane transplantation. The corneal perforated again one year later,needing penetrating keratoplasty. Wear of therapeutic soft contact lens became difficult due to progression of impaired lid closure and scarring of conjunctival sac.
