過去6年間に当院の新生児集中治療室に入院し,眼科的に検索した193例について,未熟児網膜症の危険因子を検索した。未熟児網膜症は49例(25.4%)に発症し,36例(18.7%)が重症でレーザー治療を要した。未熟児網膜症の発症と病期に有意に関連する因子は,外科治療,動脈管開存症,出生時体重,人工換気期間,在胎週数,酸素投与期間であった。未熟児網膜症に対する治療の適応に有意に関連する因子は,人工換気期間,在胎週数,外科治療,1分のApgar scoreであった。Ⅱ型網膜症の発症に有意に関連する因子は,外科治療,酸素投与期間,人工換気期間,輸血回数であった。
We reviewed a consecutive series of 193 infants admitted to our Neonatal Intensive Care Unit. All the cases received ophthalmological examinations. ROP developed in 49 infants(25.4%),of whom 36 cases(18.7%)had severe ROP needing laser treatment. Following 6 items were significantly related to occurrence and stage of ROP(p<0.05):general surgery,patent ductus arteriosus,low birthweight,duration of artificial ventilation,shorter gestational age,and duration of oxygen inhalation. Following 4 items were significantly related to laser treatment:duration of artificial ventilation,gestational age,general surgery,and one-minute Apgar score. Following 4 factors were significantly related to typeⅡ ROP:general surgery,duration of oxygen inhalation,duration of artificial ventilation,and frequency of blood transfusion.
