Purpose:To evaluate the dose-dependent effect of trans-Tenon infusion of triamcinolone for diffuse diabetic macular edema. Cases and Method:This prospective study involved 45 eyes of 41 patients with diabetic macular edema and intact vitreous. As the initial treatment,13 eyes received 20mg and 32 eyes received 40mg of triamcinolone acetonide. The former group was followed up for 3 to 7months,average 4.5months. The latter was followed up for 3 to 16months,average 6.8months. Results:Foveal thickness decreased by 20% or more in 9 eyes(69%)of 20mg group and in 24 eyes(75%)of 40mg group. Visual acuity improved by 2 lines or more in 5 eyes(38%)of 20mg group and in 20 eyes(63%)of 40mg group. Intraocular pressure rose to 21mmHg or over in 1 eye(8%)in 20mg group and in 4 eyes(13%)of 40mg group. Conclusion:Trans-Tenon infusion of triamcinolone acetonide was effective in both groups. Macular edema tended to persist in 20mg group. Ocular hypertension was more frequent in 40mg group.
