 We performed vitreous surgery on 14 eyes of macular edema with intraoperative injection of triamcinolone into the vitreous and sub-Tenon capsule. The series comprised 8 eyes of 8 cases of central retinal vein occlusion(CRVO)and 6 eyes of 4 cases of diabetic retinopathy,consisting of 6 eyes of 5males and 8 eyes of 7 females. Their ages ranged from 50 to 76 years,average 67 years. We injected 4mg triamcinolone into the vitreous and 20mg into sub-Tenon capsule. Eyes of CRVO received additional radial optic neurotomy. Diabetic eyes received photocoagulation. One month after surgery,all the eyes showed decreased macular edema with improved visual acuity. The foveal thickness averaged 600±150μm before surgery and 216±57μm one month after surgery. The difference was significant(p<0.0001). Macular edema recurred in 4 eyes of CRVO(50%). The findings show that early resolution of macular edema is induced by simultaneous injection of triamcinolone into the vitreous and sub-Tenon capsule. Recurrence of macular edema in CRVO remains a major problem.
