　症例は80歳男性で，発熱，意識障害，全身痙攣発作で発症した。頭部magnetic resonance imaging（MRI）のfluid attenuated inversion recovery（FLAIR）像で両側の大脳辺縁系が高信号を示し，血清と髄液のウイルス抗体価より単純ヘルペス髄膜脳炎と診断した。発症約6週間後から両側前頭葉と側頭葉の白質にFLAIR像で高信号を示す病変が出現した。その病変は左右対称性に拡大し，新皮質を除いて前頭葉，側頭葉の広範囲に及んだ。髄液中のmyelin basic protein（MBP）は入院時0.9ng／ml（正常：＜4ng／ml）であったが，発症から約2週間後に233.9ng／mlと極めて高い値を示した。6週間後は25.4ng／ml，2カ月後は18.4ng／ml，3カ月後は7.4ng／ml，6カ月後4.3ng／mlと徐々に低下した。本症例の大脳白質病変は，MRI所見と髄液中のMBPの経時的変化より単純ヘルペス髄膜脳炎に続発した脱髄病変の可能性が示唆された。

An 80-year-old male without abnormal past medical history presented with coma, general seizures, and fe-ver subsequent to abnormal behavior. The pressure of the cerebrospinal fluid (CSF) elevated (13.5-20.5cm H₂), and CSF examination revealed pleocytosis with predominant mononuclear cells (80-879/mm³) and ele-vated protein level (32-130 mg/dl). DNAs of herpes simplex virus (HSV) type 1 and 2 in CSF were not confirmed by polymerase chain reaction method in the acute phase.
