不顕性に経過したprevertebral abscess—急性化膿性髄膜炎と敗血症性ショックをきたした1剖検例

Prevertebral abscesses with a Protracted Insidious Clinical Course and Subsequent Lethal, Acute Pyogenic Meningitis and Septic Shock

　約3カ月間にわたり軽い後頸部痛のみを訴えていた66歳の男性が，急激な黄色ブドウ球菌髄膜炎を発症し，その第2病日に敗血症性ショックで死亡した。剖検にて，頸椎，胸椎椎体腹側の結合織性被膜に覆われたprevertebral abscessと，これと連続する化膿性椎体・椎間板炎が広範に認められた。膿瘍の器質化の程度から，化膿巣はまず上位頸椎腹側に形成され，その後椎体腹側を上下に走るprevertebral spaceに沿って尾側に進展したものと考えられた。さらに化膿巣は隣接する椎体・椎間板へと波及し，ついには敗血症から化膿性髄膜炎に至ったものと考えられた。Prevertebral abscessは，広範な進展をきたしていても不顕性に経過し，激烈かつ重篤な敗血症をきたすことがあるため，できるだけ早期に発見，治療されるべき病態である。

This report concerns a 66-year-old man suffering from prevertebral abscesses with a protracted insid-ious clinical course and subsequent lethal and acute pyogenic meningitis. The patient had a three-month history of mild neck pain, and died as a result of septic shock due to staphylococcus aurcus (meth-icillin-susceptible) infection two days after admis-sion to the hospital. At autopsy, abscesses encap-sulated by fibrous connective tissue were found on the ventral surfaces of the cervical and thoracic regions of the spine.
