対側の上行性感覚障害で発症した中脳背外側グリオーマの1例

Brain Stem Glioma Presenting with Contralateral Sensory Impairment of Ascending Nature

　上行性の対側温痛覚障害で発症した中脳背外側グリオーマの1例を報告した。症例はvon Reck—linghausen病を有する38歳女性。左側下肢より徐々に上行する温度覚障害を主訴に入院した。MRI上，右中脳背外側の表層部（主に下丘部）で，四丘体槽に突出する長径2cmの腫瘍を認めたため摘出術を行った。術前は左下肢から左上肢へと広がっていた温痛覚障害が，術後はさらに上行し，左顔面まで含む全感覚脱失となった。感覚障害が一過性に上行したあと，術後放射線治療を加え，2カ月後には下肢のみの障害となり固定した。術前の進行とは逆方向の下行性の改善であった。表層部の腫瘍の増大と共に症状が下肢から上行性に進行し，治療後は逆方向の改善という臨床経過より中脳背外側に位置する外側脊髄視床路にもlaminationがあることが示唆され，後方から下肢→体幹→上肢の順に並んでいると思われた。

A case of brain stem glioma, presenting with impairment of contralateral pain-temperature sense of ascending nature, is reported. A 38-year-old woman with documented neurofibromatosis type 1 (NF-1) was admitted to our hospital for treatment, complaining of diminished pain-temperature sense in the left lower extremity. On admission, the symp-tom was first evaluated to be due to cervical myelopathy although motor involvement was absent. An MRI, myelography and CT-myelogra-phy of the cervical spine were done, demonstrating no abnormality.
