　症例は40歳男性。感覚障害が病側口周囲と対側下腿部に限局し，頭部MRIにより延髄外側梗塞と診断した1例を報告した。この特異な感覚障害の分布について，延髄外側梗塞を中心とした文献的考察を行った。本症例の口-下腿部に限局した交叉性感覚障害は，延髄外側梗塞のfar-lateral lesionまたはinfero-lat-eral typeに最も類似すると考えられた。本症例で認められたcrossed oral-pedal sensory patternの感覚障害は，延髄外側梗塞による感覚異常が多彩な分布様式を呈しうることを示唆している。したがって，特異な感覚障害分布を有する症例では，延髄外側部の病巣を十分に考慮することを強調したい。

We report a 40-year-old man with hypertension and diabetes mellitus, who had crossed oral-pedal sensory disturbance in lateral medullary infarction. He suddenly developed dysesthesia in the right mouth and the left leg. His blood pressure was 150/ 90 mmHg. Neurological examination showed Homer's sign in the right eye and horizontal nystag-mus. Sensory function revealed decreased tempera-ture, hypalgesia and dysesthesia in the right mouth and the left leg. Vibratory and position sense were normal.
