水頭症手術後にパーキンソン様症状を呈し，L-dopaの投与にて症状の改善が得られた中脳水道狭窄症の2例

L-dopa Effective Parkinsonism Appeared after Shunt Revision of the Aqueductal Stenosis:Report of Two Cases

　今回われわれは，短期間にシャント不全を繰り返した中脳水道狭窄症による水頭症の2例を経験した。両症例とも水頭症手術を繰り返すうちにparkinsonismが出現してきた。そしてその症状はdopamine受容体刺激薬や塩酸amantadine，L-dopsの投与に反応せず，L-dopaを投与することで改善が得られた。本症例を通じて，シャント再建術後にparkinsonismが出現するメカニズムとして，繰り返しの第3脳室の拡大縮小に伴う黒質線条体路の振盪による可逆性機能障害が関与しているのではないかと思われた。

We present a two cases of parkinsonism that ap-peared after shunt revision in obstructive hydrocepha-lus patients. Case 1 was a 59-year-old man and case 2 was a 32-year-old woman. Parkinsonism appeared in both cases after 3 or 4 shunt revisions within a period of three months. Treatment with bromocriptine, aman-tadine and L-dops had no effect, but the parkinsonism dramatically improved with administration of L-dopa.
