Serial scans of the moving state of intrathecally injected radioisotopes, so-called radioisotope cister-nography and ventriculography, has recently become an indispensable auxiliary diagnostic method in the field of neurosurgery. We have also injected a new radiopharmaceutical, 169YbDTPA intrathecally and conducted dynamic observation using GAMMA CAMERA.

This method was found to be of great value in the diagnosis head trauma, congenital communicat-ing (or non-communicating) hydrocephalus, cerebro-vascular disease (mainly aneurysm or arteriovenous malformation leading to subarachnoid bleeding), leakage of cerebrospinal fluid, intraventricular tumor, leptmeningeal cyst and porencephaly, as well as secondary circulatory disturbance of cerebrospinal fluid following these disease (normal pressure hydro-cephalus).

Despite a rather long physical half-life of 169YbDTPA, 32 days, biological half-life is rather short. Compared to 131I, so far used in this kind of test, administration of larger dose is possible. Since no risk for aseptic meningitis is involved, this radioisotope appears to be adequate for this method. Through combining 1600 word memory system with GAMMA CAMERA, clearance curve in the lateral ventricle was prepared in normal pressure hydro-cephalus and indication for operation was decided from the specific pattern.
