Positive contrast ventriculography is one of the most valuable diagnostic procedures for the lesion in the ventricle, pineal region, midbrain and posterior fossa. However, its parformance necessitates most careful indication because of its serious complications such as pressure cone or hazardous reaction to the contrast medium injected into the lateral ventricle.

A new method of the positive contrast ventriculo-graphy was reported by Portera-Sanchez et al. who made an emulsion by vigorous shaking of mixture of 2. 5 ml of Pantopaque, 15 ml of cerebro-spinal fluid and 2. 5 ml of air and rapidly injected 20 ml of this emulsion into the lateral ventricle. Utilizing Myodil, the authors have employed this method on 17 patients since 1964 and confirmed superiorities of this maneuver to the old method that required difficult posturing under fluoroscopy. The advantages include the simpli-city of the technique, the ability to outline the entire patent ventricular system and to survey the relief of the ventricular wall with a single injection.

History of the positive ventriculography was re-viewed and its indication discussed. The technique standardized from own experience was presented in detail and roentgenograms of 8 patients with thalmic tumor, midbrain tumor, pineal body tumor, 4th ven-tricle tumor, craniopharyngioma, malignant pituitary adenoma and cerebellar tumor were illustrated.
