I.緒言
硬膜下hygromaは,硬膜下に髄液様の液体の貯留したものでsubdural effusion,またsubdural hydromaともいわれ,主として頭部外傷に起因する。この疾患に関しては,1894年にMayo1)がbrain cystとして報告したのが最初である。1945年Wycis2)は自験例7例を加えた99例について総括的報告を行なつている。近年頭部外傷の増加とともにその報告例も増加しつつあるが,頭部外傷歴は認められない。
Report on a subdural hygroma with local bulging of the skull in a girl aged five. Roentgenograms of the skull showed thinning and bulging of the left temporal squama of the skull and the adjacent parts of the frontal and parietal bones. The middle fossa was enlarged forwards and the lesser wing of the sphenoid was elevated. Left carotid arteriogra-phy indicated an avascular area between the brain and the skull. Left temporoparietal craniotomy was performed. The dura mater was bluish and on op-ening it there was a large collection of clear fluid in the middle fossa. The inner wall of the hygro ma was completely removed and the deformed tem-poroparietal bone flap was repaired to be flat. Postoperative course was satisfactory. Left carotid arteriography performed 51 days after surgery revealed no avascular area and suggested swelling of the cerebral hemisphere.
