A 48-year-old woman was admitted to Kansai Rosai Hospital on October 25, 1976, because of a disturbance of consciousness. For about 5 months prior to this admission she complained of intermittent headaches and "heating" in her left ear.

Neurological examination on admission: The patient was in a drowsy state, and had bilateral papilledema and a stiff neck. Hoffmann's sign was positive on her right side. A loud pulse-synchronous bruit was audible over the left mastoid area.
