We have reported a case of spasmodic torticollis against medical treatment but alleviated completely by bilateral cryogenic basal ganglia surgery.

The patient is a 31 year old man who started to notice of difficulty in motion of the right thumb about a year and 6 months ago, and of clonic head involuntary movement backwards started about 3 months ago, and then he began to feel of head lean-ing to the left side.

Although the patient admitted to a certain hospital to undergo the medical treatment, he could not any improvement. So he entered into our clinic. Bilat-eral stereotaxic cryogenic surgeries were performed upon this patient with 4 months interval, and voa lesions were made in the VL nuclei of the both thalamus. Immediatly after the second surgery, the patient was completely alleviated and began to be able to move the right hand voluntarily.

We have given some discussions about mechanism of arising of torticollis and it's treatment, and have also given some data of EMG examination in this report.
