Sixty-seven years old male. In the autopsy of this case which was firstly considered as senile dementia and latar diagnosed as general paresis with the spinal fluid examinations, be-sides the findings specific to general paresis, Alzheimer's change of neurofibrilles and Sim-chowicz's granular degeneration were found.

Histologically, perivascular cell infiltration, multiplication and hypertrophy of glia cells and deterioration of nerve cells in cerebral cortex were remarkable and furthermore a rather lar-ge amount of myelophoride was noticed. Spir-ochaetes were detected in cerebral cortex. At the same time intense fatty degeneration in wi-de-spread nerve cells and Alzheimer's change of neurofibrilles and Simchowicz's grobe-gran-ular degeneration were found in frontal and lateral lobes and Ammon's horn. The combi-nation of these changes of neurofibrilles in the brain of general paresis is very rare and Sek-ine's report is the only one in Japan consern-ing this combination. In the cortex of frontal lobe of this case, cell groups which simulated miliary gummata were found but no giant cells and necrotic foci were found in these lesions. Simultaneously, the Tsujiyame's staining me-thod of nerve fibres with pyridin silver was introduced which is believed to be superior to the Bielschowskyis method.
