 In this study on clinical diagnosis and managements of the psychiatric conditions of geriatric patients, we included 115 patients aged over 90 years. Of them, 76 patients were followed up over the next 18 months.

 Correct diagnosis of psychiatric ailments in geriatric patients is very important to support their treatment and nursing care and also to help them achieve an improved quality of life (QOL). Clinical diagnosis of dementia is challenging, especially in geriatric patients. We studied the clinical diagnostic criteria of dementia by using mini-mental state examination (MMSE), magnetic resonance imaging (MRI), and single-photon emission computed tomography (SPECT) as well as CDR and DSM-Ⅳ. We found that the MMSE threshold was 14/15 for the oldest older patient in this study group.

 SPECT images showed that decreased regional cerebral blood flow in the angular gyrus might be related to the development of dementia. The voxel-based specific regional analysis findings system for Alzheimer's disease (VSRAD) scores and SPECT may be related to the MMSE scores;however, the results are not useful for clinical diagnosis of dementia in patients aged over 90 years.

 Several patients were incorrectly diagnosed with dementia. Prevention of cognitive decline is possible in some patients by improving the environment of the patients or by improving the coping strategy.
