　左室拡張障害が基本病態である肥大型心筋症（HCM）について，運動負荷時の心拍出量（CO）の変化の特徴と，これが運動耐容能に及ぼす影響を検討するため，左室流出路狭窄のないHCM 12例にトレッドミル運動負荷試験を行い，アセチレン再呼吸法を用いて心拍出量と酸素摂取量（V_O2）を同時に測定した．結果はHCMでは心係数（CI）は安静時，軽度負荷時は正常者よりやや低値で，その差は中等度〜亜最大負荷で増加した．この差は1回拍出係数（SI）によるものと考えられた．V_O2もCIとほぼ同様の傾向を示し，両者の間には正の相関関係が認められた．また回復期では正常者と比較してCI，V_O2共に安静時への回復が遅延した．以上よりHCMでは運動負荷中，1回拍出量（SV）によるCOの増加制限があり，このため活動筋に対する酸素供給が心原性に制限されていると考えられ，これは中等度〜亜最大負荷で著明となり，運動耐容能低下の一因になると思われた．

In order to investigate the relation between exercise intolerance and cardiac output (CO) increase during exercise in patients with hypertrophic cardiomyopathy (HCM), submaximal treadmill exercise (Sheffield pro-tocol) with simultaneous measurement of CO and oxy-gen uptake (_VO2) by acetylene rebreathing method were performed in twelve patients with HCM who had no demonstrable left ventricular outflow obstruction and fifteen control men. The measurements were made at rest, during exercise and recovery.
