ヒス束内Wenckebach現象を呈した1例

A case report of intrahisian Wenckebach phenomenon

　ヒス束内Wenckebach現象は比較的まれであり^1〜7），房室結節内Wenckebach現象が生理的反応であるのに対して病的反応と考えられている^2,8）。最近我々は，臨床経過中に，1：1房室伝導，escape-capture bigeminy，Wenckebach型房室ブロック，2：1房室ブロックおよび房室解離を呈し，電気生理検査でそれらのリズムが再現され，ヒス束内ブロックによることが示された症例を経験したので報告する。

A 78-year old woman with a syncopal attack and exertional dyspnea, admitted to our hospital. Her ECGs showed 1:1 atrio-ventricular (AV) conduction with PR-interval of 0.18-0.22 sec., Wenckebach type second-degree AV block, escape-capture bigeminy, 2:1 AV block and AV dissocia-tion with junctional rhythm. She underwent an electrophysiologic study and these ECG findings were all reproduced by atrial pacings and showed to be due to intrahisian block. The present case may be the second reported case of intrahisian Wen-ckebach phenomenon in Japan.
