A case of torsades de pointes developed in a patient with atrio-ventricular (A-V) block was reported. The serial ECG recordings revealed intermittent A-V conduction for about one year. Right bundle branch block seemed to be present constantly and left bundle branch showed conduction which pro-gressed to the third degree block. The change in QRS morphologies were explained by the shift of the subsidiary pacemaker site. The pacemaker shift might be related to the development of torsades de pointes in this case.

The female preponderance of torsades de pointes complicating A-V block was also stressed from a review of literatures.
