  • Look Inside

A thirty-six year-old woman with severe chronic anemia (RBC 110×104/mm3, Hb 2.0g/dl, Ht 8%) due to myoma of the uterus, had an attack of torsades de pointes after the correction of anemia and hysterectomy. On admission, her CTR was 55 % and "Cabot-Locke" murmur was recorded phonocardiographically. No valvular lesion nor other organic heart disease was revealed by echocardiography without slight thickening of posterior wall of the left ventricle. The serum potassium level was 3.4mEq/L and the serum calcium level was 3.7 mEq/L. Torsades de pointes occurred with QT prologation (0.64 sec) and sinus bradycardia (43 bpm), and controlled with ventricular pacing (80 ppm) completely. A slight prolongation of QT interval was seen in the ECG recorded after a month.

During and after a correction of severe chronic anemia, one should not only pay attention to serum electrolytes, but also monitor ECG.
