Abstract Purpose:To report a case in which severe sub-surface nano glistening(SSNG) compromised visual function after a long-term postoperative period, with subsequent improvement in visual function following intraocular lens(IOL) exchange.

Case:A 71-year-old man with a history of bilateral glaucoma surgeries performed over 50 years ago underwent cataract surgery with intraocular lens implantation in the left eye in 1999(IOL model:MA60BM, Alcon, +17.0D) and in the right eye in 2004. The left eye experienced progressive visual function decline for several years, with a recent marked worsening that prompted a referral to our hospital. At the initial visit, the corrected visual acuity was 1.2 in the right eye and 0.09 in the left eye. The left eye exhibited severe SSNG, with a corneal endothelial cell density of 1,750 cells/mm2. After obtaining thorough informed consent, including a detailed discussion of the risks associated with IOL exchange, the patient opted for surgery, which was performed in July 2023. During surgery, the IOL was strongly adhered to the capsular bag, necessitating the removal of both the IOL and the capsular bag. Scleral fixation of a new IOL(NX-70S, Santen, +15.5D) was performed. Postoperatively, the corrected visual acuity in the left eye improved to 0.15, and the patient reported subjective improvements in visual function. Examination of the extracted IOL revealed a light transmittance reduced to approximately 40%.

Conclusion:We report a case in which severe SSNG caused significant visual impairment 24 years after cataract surgery. Thus, IOL removal and exchange are effective in restoring visual function in advanced SSNG cases.
