Abstract Objective:To report the efficacy of extracapsular cataract extraction using L-shaped scleral pocket incision in three cases of microcornea.

Methods and Cases:The method involved making an L-pocket incision1) as reported by Ota et al. The nucleus was delivered with an horizontal incision of 9 mm and a vertical incision of 2 mm after continuous curvilinear capsulorrhexis with 2 relaxing incisions. Case 1 is a 64-year-old woman. It has been pointed out Peters anomaly. The corneal vertical length was 6.7 mm and the horizontal length was 7.4 mm. Preoperative visual acuity was vs=(30 cm/n. d.). The patient was diagnosed with a case of grade 4 cataract in the left eye. Postoperative visual acuity was vs=(0.01). Case 2 is a 78-year-old woman with microphthalmos atypical coloboma. The corneal vertical length was 7.7 mm and the horizontal length was 8.0 mm. Preoperative visual acuity was vd=(0.01). The patient was diagnosed a case of cataract of grade 4 cataract in the right eye. Postoperative visual acuity was vd=(0.15). Case 3 is a 63-year-old woman with microphthalmos and coloboma. The corneal vertical length was 8.2 mm and the horizontal length was 8.8 mm. Preoperative visual acuity was vs=(30 cm/m. m.). The patient was diagnosed with a case of grade 5 cataract in the left eye. Delivery was difficult due to the hard and large nucleus, and a 2-mm transverse incision was required. Postoperative visual acuity was vs=(0.02p). Postoperative visual acuity improved in all cases.

Conclusion:Using the L-pocket incision in pathological cases with microcornea, ECCE could be performed with an incision smaller than that required routinely, In cases with hard lens nucleus, however, it is necessary to modifyoperative procedure flexibly including the expansion of incision wounds.
