 To elucidate the natural history of endoscopic findings in autoimmune gastritis (AIG), we retrospectively evaluated chronological changes in 15 patients with AIG who underwent serial esophagogastroduodenoscopy (EGD) for more than 10 years. At the initial EGD, most cases corresponded to early-stage AIG (AIG-AS stage 1 or 2). Characteristic findings included erythematous and edematous changes of the areae gastricae and longitudinally arranged pseudopolyps. Atrophic changes progressed in a patchy and uneven manner, initially involving the upper lesser curvature of the gastric body and the fundus. At the final EGD, AIG-AS stage 3 was observed in the majority of patients. Hyperplastic polyps frequently appeared prior to the development of adherent mucus. Recognition of these chronological endoscopic changes is important for the early diagnosis of AIG.
