要旨

　In Japan, the incidence of gastric cancer is on the decline due to a significant reduction in Helicobacter pylori infection rates across generations, which is the primary cause of the disease. Moreover, the mortality rate from gastric cancer has markedly decreased because of the increased number of early-stage cancers. In contrast, the prevalence of gastric cancer continues to rise, primarily due to the aging of the population and the broader adoption of endoscopic gastric cancer screening, which has led to the detection of more cases. However, this prevalence is expected to decrease in the future. As the number of new gastric cancer cases among younger individuals continues to decline, it is anticipated that the management of gastric cancer will increasingly focus on elderly patients. Based on the limited available evidence, it is speculated that the progression from early to advanced gastric cancer typically requires several years, and the impact of age on the speed of progression appears to be minimal. Furthermore, treatment outcomes for elderly patients-particularly those with cardiac gastric cancer-are generally poor. Therefore, when gastric cancer is diagnosed in elderly individuals, treatment decisions should be made carefully, weighing the potential benefits against the individual risks.
