要旨
十二指腸癌は従来稀な疾患と考えられてきたが,近年その発見機会は増加している。欧米における十二指腸癌は緩徐に増加傾向を示す一方で,診断時に進行期で発見される症例が多く,長期生存成績は依然として不良である。全国がん登録データの解析により,本邦では欧米と比較して罹患率が高く,かつ早期段階で発見される割合が高いことが示されており,内視鏡検査の普及と診断技術の進歩がその背景にあると考えられる。また,表在性非乳頭部十二指腸上皮性腫瘍の自然史に関しては,小型の低異型度腺腫は安定した経過を示すものの,一部は高異型度腺腫や癌へ進展することが報告されており,腫瘍径や局在,形質などが進行リスクに関与する可能性が示唆されている。特に腫瘍径10mm以下などの小型の低異型度腺腫は経過観察が許容される可能性があるが,生検診断には正診率の限界があり,経過観察を選択する場合には内視鏡診断にもとづいた慎重なサーベイランスが重要である。胃型・腸型の形質ごとに臨床病理学的特徴が異なり,その自然史には未解明な点が少なくない。さらに,十二指腸腺腫・癌の確立したリスク因子はいまだ明らかではなく,今後さらなる疫学データの蓄積が期待される。
Duodenal adenocarcinoma has long been regarded as a rare disease. However, its detection has increased in recent years. In Western countries, the incidence of duodenal adenocarcinoma has shown a gradual increase, while most patients are diagnosed at an advanced stage and long-term survival remains poor. In contrast, analyses of the national cancer registry in Japan have demonstrated a higher incidence of non-ampullary duodenal adenocarcinoma and a greater proportion of cases detected at an early stage, likely reflecting widespread availability of upper gastrointestinal endoscopy and advances in endoscopic diagnostic techniques. Regarding the natural history of superficial non-ampullary duodenal epithelial tumors (SNADETs), most small low-grade adenomas follow an indolent course, whereas a subset progresses to high-grade adenomas or adenocarcinoma. Tumor size, location, and phenotypic characteristics have been suggested as factors associated with progression risk. Small low-grade adenomas ≤10 mm may be acceptable for surveillance. However, the diagnostic accuracy of forceps biopsy is limited, and careful follow-up based on endoscopic assessment is essential when a surveillance strategy is adopted. Gastric-type and intestinal-type lesions exhibit distinct clinicopathological features, and the natural history of SNADETs remains incompletely understood. Moreover, established risk factors for sporadic SNADETs have not yet been clearly identified. Further accumulation of epidemiological data is warranted.
