Pathology of autoimmune gastritis

要旨

自己免疫性胃炎（autoimmune gastritis：AIG）の病理について，診断基準における病理所見の位置づけ，診断に重要となる病理学的キーワードとその組織学的所見，内視鏡医が診断するAIGと病理医が指摘するAIGとの違いについて概説した。また，病理診断において鑑別を要する疾患・病態，ならびにAIGを背景として発生する腫瘍の臨床病理学的特徴についても整理した。近年，内視鏡医，病理医の双方においてAIGへの関心が高まっている。両者が知見・情報を共有し，診断上の注意点を理解することで，AIGの適切な診断と病態把握につながることが望まれる。

　This article provides an overview of the pathology of autoimmune gastritis (AIG), including how pathological findings are positioned in the diagnostic criteria, key pathological terms important for diagnosis and their corresponding histological features, and the differences between AIG diagnosed by endoscopists and AIG suggested by pathologists based on histological findings. It also summarizes the diseases and conditions that require differential diagnosis in pathological practice, as well as the clinicopathological features of tumors arising in the background of AIG. In recent years, interest in AIG has been increasing among both endoscopists and pathologists. Sharing knowledge and information between these two groups and understanding the diagnostic points of caution are expected to contribute to the appropriate diagnosis and pathological assessment of AIG.
