肝類上皮血管内皮腫肺転移の1例

A case of hepatic epithelioid hemangioendothelioma with pulmonary metastases

類上皮血管内皮腫（epithelioid hemangioendothelioma：EHE）は非常にまれな低悪性度の血管内皮細胞由来の腫瘍である。体内のどの部位にでも発生し，単発，多中心性発生，全身転移と様々な病型を呈する。肝EHEの画像所見として，腫瘤辺縁の陥凹，石灰化，lollipop signやtarget signが知られている。肺EHEの画像所見として，両肺気管支血管周囲の多発結節などが知られている。多発肺腫瘤・多発肝腫瘤に加え，前述の画像所見を併せて認めた場合は本疾患を鑑別に想起することが重要と考える。今回我々は，非常にまれな症例を経験したため，若干の文献的考察を加えて報告する。

Epithelioid hemangioendothelioma（EHE）is a very rare vascular tumor of low-grade malignancy that can occur in various organs. We report a rare case of hepatic EHE with pulmonary metastases. Typical MRI findings of hepatic EHE include capsular retraction, calcification, and the “lollipop” or “target” sign in the liver, whereas pulmonary EHE usually presents as multiple perivascular nodules. It is important to consider this entity in the differential diagnosis when multiple hepatic and pulmonary nodules with these characteristic findings are observed.
