尿路悪性腫瘍において扁平上皮癌はまれな組織型で，結石に伴う慢性炎症との関連が報告されている。そのため結石の既往がある症例において尿路に炎症を示唆する所見を認めた場合，腫瘍の合併に注意を要する。今回我々は，腎膿瘍を伴う腎盂の扁平上皮癌を経験したので，文献的考察を加えて報告する。

A man in his 70s with a history of renal stones presented with flank pain, hematuria, and fever. A CT scan revealed an ill-defined mass in the left kidney containing multiple low-enhancement areas. A renal stone was noted in the adjacent calyx. Additionally, an indistinct mass was observed within the renal pelvis. Nephrectomy led the diagnosis of squamous cell carcinoma with necrosis. In cases with a history of urinary stones and findings suggestive of inflammation, careful attention should be paid to the potential coexistence of tumors.
