発育過程を観察できた腎angiomyolipoma with epithelial cystsの1例

A case of renal angiomyolipoma with epithelial cysts（AMLEC）：observation of the developmental process

angiomyolipoma（AML）は，腎の良性腫瘍では最も多い。2006年にAMLの特殊型として，AML with epithelial cysts（AMLEC）という亜型が提唱された^1）2）。AMLECでは，上皮細胞に覆われた真性嚢胞と，平滑筋優位で脂肪成分の乏しい充実部をもつと報告されている^1）2）。AMLECの画像のまとまった報告は少ない。また，発育過程の報告はまれであるが，初期に単純性嚢胞が出現し経時的に嚢胞壁肥厚，壁在結節，さらに充実部が嚢胞内の大部分を示すほど増大した発育過程を含めた症例報告がある^3）。典型像に加え，発育過程の理解は，術前診断の確信度に寄与する。今回我々は，初期に小嚢胞を有する扁平な充実結節が，2年の経過で液貯留を伴う嚢胞変化優位な病変となり増大したAMLECを経験したので報告する。

Angiomyolipoma with epithelial cysts（AMLEC）is a subtype of renal angiomyolipoma characterized by cysts lined with epithelial cells and a predominance of smooth muscle with minimal fat. Typical imaging findings include simple cystic masses with areas of enhancement. The developmental process of AMLEC, observed in rare case reports, begins with a simple cyst that evolves over time into a lesion with cyst wall thickening and mural nodule formation. Understanding these processes, alongside imaging, aids in accurate preoperative diagnosis.
