フマル酸ヒドラターゼ欠損性腎細胞癌の1例

A case of fumarate hydratase–deficient renal cell carcinoma

フマル酸ヒドラターゼ（fumarate hydratase：FH）欠損性腎細胞癌は，FH遺伝子の変異によって生じるまれな亜型である。局所進行性で，転移しやすく，予後不良といった点は多くの文献に共通しており，悪性度の高い腫瘍と考えられている。疾患概念が確立されてからまだ日が浅く，画像的な知見は限られているが，数少ない報告によると大小の多彩な嚢胞を形成することが多い。今回我々は，充実部を伴う嚢胞性腫瘤を呈したFH欠損性腎細胞癌の1例を経験したので，文献的考察を加えて報告する。

A man in his 70s was incidentally found to have a solitary left renal tumor. The tumor had a solid and cystic component with a maximum diameter of 5 cm. The contrast enhancement of the solid area showed weaker in the nephrographic phase compared to the corticomedullary phase. Immunohistochemical staining led to the diagnosis of fumarate hydratase（FH）–deficient renal cell carcinoma（RCC）. Although clear cell carcinoma is more common in cystic RCCs, FH–deficient RCC should be considered when the solid area does not show marked vascularity.
