Macklin effectは肺野の小血管および関連する気管支に隣接する気腫と定義され，胸腔内圧上昇に伴う終末肺胞の破裂を反映した所見と考えられている。今回我々は，Macklin effectをCTで認めた翌日に特発性縦隔気腫を発症した若年成人男性の症例を経験した。特発性縦隔気腫の発生メカニズムとの関連とともに報告する。

We report a case of spontaneous pneumomediastinum in a young adult male. Retrospectively, chest CT taken the day before onset of spontaneous pneumomediastinum revealed linear emphysema in the peripheral area of the left upper lobe. This finding is known as the Macklin effect, which is thought to reflect terminal alveolar rupture. In this case, serial imaging studies demonstrated that the Macklin effect preceded the development of spontaneous pneumomediastinum, emphasizing the importance of careful observation and repeated imaging evaluations.
