主幹動脈閉塞に対する血管内治療による急性期血行再建術は標準的治療となり，ますますその重要性は増している^1）2）。しかし，症例数の増加とともに難渋する症例を経験することも増えてきた。

Thrombus retrieval therapy for acute main artery occlusion has become the standard of care and is widely used, but there are some cases that are difficult to treat. In this case, we used a diagnostic catheter as an intermediate catheter and continued the trans–femoral approach to a case in which a conventional guiding catheter with a balloon could not reach the internal carotid artery. This method may be useful for patients who are difficult to approach.
