特徴的な画像所見を呈した下腿のメトトレキサート関連リンパ増殖性疾患の1例

A case of methotrexate–associated lymphoproliferative disorders involving the lower extremity exhibiting characteristic imaging findings

メトトレキサート関連リンパ増殖性疾患（metho-trexate–associated lymphoproliferative disorders：MTX–LPD）はメトトレキサート投与下の免疫不全を背景に発症するリンパ増殖性疾患（lymphoproliferative disorders：LPD）である。画像所見は非特異的だが，内部壊死が示唆された症例がこれまでに複数報告されている。今回我々は，関節リウマチ（rheumatoid arthritis：RA）の加療中に左下腿に腫瘤を認め，MRIとタリウムシンチグラフィで特徴的な画像所見を呈したMTX–LPDの症例を経験したため報告する。

Methotrexate–associated lymphoproliferative disorders （MTX–LPD） represent infrequent benign lymphoid proliferations or malignant lymphomas arising in patients exhibiting immunodeficiency due to methotrexate therapy. We present a case of MTX–LPD originating from the lower extremity, wherein contrast–enhanced MRI and thallium–201 scintigraphy indicated necrosis within the mass, subsequently confirmed through pathological examination. To the best of our knowledge, this is the first report describing the contrast–enhanced MRI and thallium–201 scintigraphy characteristics of MTX–LPD originating in the muscular tissue.
