 Endovascular neurosurgery has been established as a first-line therapy for cerebrovascular diseases. Although many endovascular procedures can be performed in conventional angiosuites, a sterile and well-prepared environment is needed to perform combined surgical exposure and endovascular procedures for complex neurovascular diseases, such as thrombosed giant aneurysms. Therefore, we established a hybrid operating room (OR) to treat cerebrovascular disease. The first hybrid OR was established in 2003 using a biplane DSA system with a surgical OR table system. A new concept involving the implementation of industrial robot technology in angiography systems was developed in collaboration with Siemens, and a new image-guided surgery environment was established (Robotic DSA). Our hospital currently has three hybrid operating rooms (one with a biplane angiography machine and two with a robotic DSA). This study describes the utilization of hybrid operating rooms for treating cerebrovascular diseases and their disadvantages.
