腹部内臓動脈瘤の原因として，中膜の分節状の融解を本態とするsegmental arterial mediolysis（SAM）という概念が知られている。まれな疾患とされ，特に再発例の報告は少ない。治療方針や治療後の経過観察に関する定まったものはない。今回，左右胃大網動脈（gastroepiploic artery：GEA）に異時性にSAMを発症し，無症候性だったが，造影computed tomography（CT）で偶然発見され，経カテーテル動脈塞栓療法（transcatheter arterial embolization：TAE）にて治療した1例を経験したので報告する。

We present a 70’s male case of metachronous onset of segmental arterial mediolysis（SAM）in the right and left gastroepiploic arteries（GEAs）. Although he had no symptom at the first and second onsets, radiologists had diagnosed SAM referring to the aneurysm morphology on contrast-enhanced computed tomography and angiography. Transcatheter arterial embolization（TAE）was one of the treatment options for SAM. However, there was no established guideline for follow-up examinations and treatment for SAM. We would like to report our case with some literature review.
