悪性大静脈症候群に対する大静脈ステント留置術は，2019年から保険診療として施行可能となっている。術者要件として日本IVR学会認定専門医（放射線カテーテル治療専門医）が挙げられているが，手技自体の技術的難易度はさほど高くないため，基本的なIVR技術を習得していれば手技の施行は可能である。しかし，安全かつ適切な手技の施行には，適応判断ならびにデバイスの特徴，トラブルシューティングも含めた知識が必要となる。本稿では，悪性大静脈症候群に対する大静脈ステント留置術について，適応，基本的な手技の手順，合併症について，典型例を供覧しつつ述べる。

Stenting for vena cava syndrome caused by malignancy has been covered by insurance in Japan since 2019. The vena cava stent available in Japan is the SPIRAL RELIEF STENT^® by Cosmotec Co., Ltd. Vena cava stenting is indicated for both the superior and inferior vena cava. Stenting of the vena cava is not technically difficult, but serious complications such as fatal pulmonary thromboembolism, heart failure, and stent migration can occur. The decision on the indication requires multidisciplinary consideration. In addition, a few key technical points can reduce the risk of complications related to stent placement.
