脊髄硬膜外血腫（spinal epidural hematoma：SEH）は比較的まれな疾患である。特に随伴病変なしで椎間関節に沿って生じた脊髄硬膜外血腫は，2003年にNishidaら^1）により脊椎の “joint apoplexy” として初めて報告された。今回我々は腰椎のjoint apoplexyと考えられた脊髄硬膜外血腫の1例を経験したので，文献的考察とともに報告する。

A 60s man presented with pain in the right lower extremity. MRI showed a hemorrhagic epidural mass continuing from the L4/5 right facet. CT showed the facet degenerative changes and erosion. Erosion without sclerotic margin of the L4 posterior vertebral surface was also revealed. The patient underwent mass resection and the pathological diagnosis was chronic epidural hematoma. This spinal epidural hematoma with the facet changes was considered as joint apoplexy.
