Theranostics：learning from the international community and japanese perspective

PET診療と核医学治療が核医学診療の二本柱になりつつある。核医学治療においては，PET，シンチグラム・SPECTで画像化しつつ，患者選択，効果予測，効果判定，経過観察を治療と同一分子で核種のみを変えて行うことが可能であるtheranosticsの概念が広がっており，世界的に日々重要度を増している。ひるがえって国内をみた場合，核医学治療の環境整備が人的にも施設的にも法的にも不十分であるのが現状であり，世界に大きく立ち後れている。今後，国内の患者利益を確保するためには，アカデミア，企業，関連行政，患者が協調して，新規治療の国内導入・開発を図ることが求められる。

Clinical PET and targeted radionuclide therapy are considered to be major roles of recent nuclear medicine practices in daily patient management. The concept of theranostics, which provides clinicians with valuable information at every step including patient selection, prediction and judgement of therapeutic efficacy and follow-up after treatment by means of diagnostic images using the same molecule as therapeutics, has been widely accepted. The situation in Japan is far behind the international development in all aspects of human resources, infrastructure and legislation. In order to secure benefits of patients, collaborative works among academia, industry, authority and patient groups are highly needed.
