日本における粒子線治療は，臨床研究を経て先進医療となり，2015年には年間約5,000例（陽子線治療3,000例，重粒子線治療2,000例）に実施されるようになった（図1）。2016年からは，先進医療Aとして実施されている粒子線治療は，日本放射線腫瘍学会が定めた全国統一方針に従って実施されるとともに，特に重点的な評価が必要な分野に関しては先進医療Bとして実施されるようになった。また，2016年および2018年の保険改定では，いくつか重要な疾患が保険収載された。2018年4月には，国内に14箇所の陽子線，5箇所の重粒子線治療施設が稼働中であり，実施予定施設は現在でも増加傾向にある。

Particle beam therapy has been performed in Japan mainly as advanced medical care（AMC）until 2016. However, the current therapy is advancing the process from AMC to national medical insurance treatment. Japanese Society for Radiation Oncology（JASTRO）started several clinical trials for the important diseases as AMC-B, and defined identical rule for indication & treatment method in particle beam therapy, and the all data should be registered in the multi-institutional data-base（AMC-A）. From the spring of 2018, not only pediatric cancer, but also bone & soft tissue sarcoma, non-squamous cell carcinoma in head & neck, and localized prostate cancer were permitted to be treated as national insurance coverage in Japan. JASTRO should make an effort to accumulate scientific evidence for this therapy.
