薬剤関連顎骨壊死（medication-related osteonecrosis of the jaw：MRONJ）は，ビスホスホネート関連顎骨壊死（bisphosphonate-related osteonecrosis of the jaw：BRONJ）や抗RANKL抗体製剤デノスマブ関連顎骨壊死（donosumab-related osteonecrosis of the jaw：DRONJ），血管新生阻害薬による顎骨壊死を含む総称として米国顎顔面外科学会（American Association of Oral and Maxillofacial Surgeons：AAOMS）が提唱している名称である^1）。日本の顎骨壊死検討委員会ポジショニングペーパーでの（血管新生阻害薬を含まない）骨吸収抑制剤関連顎骨壊死（antiresorptive agent-related osteonecrosis of the jaws：ARONJ）に相当する^2）。AAOMSの診断基準においては臨床所見・情報によってなされ，診断基準項目に画像所見の記載は含まれていない^1）。その一方，臨床所見のみでは粘膜下における病変の詳細な進展範囲評価は困難であり^3）4），実臨床としてCT，MRIが診断，病変の進展範囲，活動性の評価などを目的に施行される。本稿ではARONJ/MRONJのCT，MRI所見に関して文献学的考察および我々の検討を交えて概説する。

Although CT and MRI of Antiresorptive agent-related osteonecrosis of the jaw（ARONJ）/medication-related osteonecrosis of the jaw（MRONJ）show nonspecific findings as seen in active osteomyelitis, there are some characteristics as follows；unilateral maxillary sinusitis adjacent to ipsilateral maxillary ARONJ/MRONJ on CT and MRI, DRONJ presenting larger sequestrum and periosteal reaction more frequently than BRONJ on CT, BRONJ resulting from intravenous administration of the drug presenting larger and more frequent buccolingual cortical bone perforations than BRONJ from oral administration on CT, and better diagnostic accuracy of extent of perilesional soft tissue inflammation on MR imaging than CT.
