HPV陽性中咽頭癌の嚢胞性頸部リンパ節転移と結核性リンパ節炎とのCT所見とテクスチャーパラメータの比較

CT findings including texture analysis in a differentiation between cystic lymph node metastasis of HPV-positive oropharyngeal cancer and tuberculous lymphadenitis

近年，AJCC Cancer Staging Manual第8版において中咽頭癌がHPV（human papillomavirus）陽性中咽頭癌とHPV陰性中咽頭癌に分けられた^1）。HPV陽性中咽頭癌の頸部リンパ節転移はしばしば嚢胞性リンパ節転移としてみられることがよく知られる。日常臨床において，特に既往歴がなく触知可能な頸部腫瘤を主訴とした患者に頸部嚢胞性病変の画像所見が検出されることがある。リンパ節領域に嚢胞性病変を認めた場合，放射線科医はHPV陽性中咽頭癌の嚢胞性頸部リンパ節転移とその他の頸部嚢胞性病変との鑑別にときに難渋することがある。特に結核性リンパ節炎は無痛性頸部腫瘤としての臨床所見や画像所見がHPV陽性中咽頭癌の嚢胞性頸部リンパ節転移と類似することがある。HPV陽性中咽頭癌の嚢胞性頸部リンパ節転移と結核性リンパ節炎の画像所見を比較した報告は今までにない。本検討の目的はHPV陽性中咽頭癌の嚢胞性頸部リンパ節転移と結核性リンパ節炎においてテクスチャー解析による内部性状を含むCT所見の比較をすることである。

We compared 13 patients（19 lesions）with cystic lymph node metastasis of HPV-positive oropharyngeal carcinoma（CLM-HPV-OPC）and 9 patients（21 lesions）with tuberculous lymphadenitis（TL）regarding age, sex, CT findings, and textural parameters. Patients with TL was significantly younger than patients with CLM-HPV-OPC（p＝0.04）. Level 1b, 4, and 5 lesions were more commonly involved in TL（p＝.005-.04）and level 2 lesions were more commonly involved in CLM-HPV-OPC（p＜.001）. Of the 41 measured texture parameters, significant differences were found in 12 ones. Although differential diagnosis between TL and CLM-HPV-OPC might be difficult on CT, age and involved lymph node level, and some texture parameters could help differential diagnosis.
