頭頸部CTにおける梨状窩虚脱の左右優位性に関する検討

Laterality in collapse of the piriform sinus on computed tomography

下咽頭は梨状窩，下咽頭後壁，輪状後部に区分され，梨状窩は下咽頭の両側前方を形成する。頭頸部CTにおいて梨状窩虚脱の左右差を認めることがある。その病的意義は不明であり，ときに虚脱側では表在性低容積病変としての梨状窩癌の判断に苦慮することもある。本研究の目的は頭頸部CTにおける梨状窩虚脱の左右優位性を検討，その臨床的意義を評価することである。

Routine CT sometimes showed predominant of laterality in piriform sinus collapse. We retrospectively evaluated CT images of 200 patients with pharyngolaryngeal symptoms who underwent endoscopy revealing no significant findings. CT images didn’t show definite focal abnormality. CT demonstrated left predominance of piriform sinus collapse（131 cases, 65.5％）, and there was a few case showing right predominance of it. Laryngoscope mostly demonstrated no laterality in piriform sinus collapse（175 cases, 94.5％）. There was no significant correlation between laterality in piriform sinus collapse and age, sex and kinds of symptoms. Hence, careful evaluation may be required to search focal abnormality when detecting right-side predominance of piriform sinus collapse on CT.
