胆管癌は近年増加傾向にある予後不良な疾患で，外科的に一括切除することが長期予後を期待できる唯一の治療法である。よって胆管癌の画像診断では，手術適応や手術方法など治療方針に影響する所見の評価が必要で，重要なポイントを認識したうえで読影する必要がある。まず非切除因子である肝転移や肺転移など遠隔転移，腹膜播種，大動脈周囲リンパ節など領域外リンパ節転移を確認し，これらの非切除因子を認めれば局所評価の重要性は低くなる。一方で遠隔転移を認めなければ，病変の範囲や周囲脈管への浸潤など局所の詳細な評価が必要となる。本稿では，胆管癌の基本的事項，形態と進展の特徴，読影ポイント，局所進展度の病期分類について概説する。

This paper reviews the basic matter, morphological classification, important point for the imaging diagnosis, and the cancer staging of extra hepatic bile duct carcinoma. Multiphase contrast enhanced CT plays a central role for imaging diagnosis. CT images of the bile duct carcinoma show abnormal enhancement and thickening of bile duct wall. Coronal images additions to axial images are useful for the evaluation of tumor invasion. And MRI（MRCP）is also useful in complimentary with CT. This review was summarized based on the educational lecture at the 53rd Autumn Assembly of the Japan Radiological Society.
