切除再建術：残存乳腺を用いた充填術

Breast Reconstruction:The Repair of Defect by Shifting Remnant Breast Tissue

　乳腺の手術に内視鏡を用いるのは傷を小さくして乳房外に置くためである．しかし術後に乳房が非対称になったり，変形が残るようでは内視鏡を併用する意味がない．したがって，より整容性が求められる．筆者らは中腋窩線上の4〜5cmの切開から吊り上げ法による乳房温存術を行っている，腫瘍占拠部位がA,B,D領域であれば乳輪縁に沿う弧状切開を追加している．乳房部分切除後の欠損部の充填はA,C領域の乳腺および脂肪組織を大胸筋から剥離し，緊張がかからないように充填する．この方法は，比較的小さな欠損にはよい適応である．また，一般外科医にも受け入れやすい術式であり，患者のquality of lifeの向上に有用である。

The aim of video assisted breast surgery is to minimize the incision and to relocate scarring outside of the breast. But if breast asymmetry or deformity remains after partial mastectomy, this surgical approach is futile.Therefore, a superior cosmetic result is ideal. We perform gasless video-assisted breast conserving surgery through a 3-4cm incision on the midaxillary line. If the tumor is in the upper inner quadrant or in the lower quadrant of the breast. a circumareola incision is necessary. After partial mastectomy, we prefer to repair the defect by shifting remnant breast tissue.
